La Fundación ICEERS (International Center for Ethnobotanical Education, Research & Service), formada por expertos y más de 200 enfermos, reclaman continuamente al Gobierno que regule el uso terapéutico del cannabis en España para dar respuesta a una realidad existente, la de personas con enfermedades crónicas que lo utilizan para mejorar su calidad de vida.
Lo han hecho en el marco del II Congreso Cannabmed que se celebró en la sala de actos del Colegio Oficial de Médicos de Barcelona (COMB) y en el Centro de Cultura Contemporánea de Barcelona (CCCB).
ICEERS defienden que más de un centenar de ensayos clínicos con cannabis y cannabinoides avalan la seguridad en el uso de esta planta. De hecho, explicaban que funciona en personas enfermas de cáncer que ya la utilizan para prevenir ciertos efectos secundarios de tratamientos de quimioterapia, como náuseas y vómitos, pero también para aliviar el dolor. También hay muestras de su uso para ayudar a disminuir los síntomas de enfermedades neurológicas como el Síndrome de Tourette, Parkinson, crisis convulsivas de la epilepsia y otras enfermedades autoinmunes como las enfermedades inflamatorias intestinales, el dolor y la rigidez de la esclerosis lateral amiotrófica (ELA) o la espasticidad asociada a la esclerosis múltiple. Hay personas que sufren todas estas afecciones que están haciendo un uso terapéutico de la planta.
La Fundación ICEERS, haciendo uso del nombre que se le ha dado al congreso, busca crear puentes entre el conocimiento tradicional sobre el uso de las plantas medicinales, la práctica terapéutica y la ciencia actual, con el objetivo de trabajar para la integración de estas plantas como instrumentos terapéuticos.
Hablamos con José Carlos Bouso, Director de Proyectos Científicos de la Fundación ICEERS, es licenciado en Psicologia por la Universidad Autónoma de Madrid y doctor en Farmacologia por la Universidad Autónoma de Barcelona. En ICEERS desarrolla estudios sobre el estado de salud y la calidad de vida de personas con enfermedades crónicas que utilizan cannabis con fines de automedicación.
¿Qué buscábais con el congreso?
La finalidad del Congreso era doble. Por un lado, allí en el Colegio de Médicos actualizar la información y el conocimiento científico que se tiene en relación al cannabis medicinal para que los médicos, que serán quién algún día si se instaura un programa de cannabis medicinal tendrán que prescribir, tengan la información más actualizada, relevante, de rigor y de calidad. El otro doble objetivo en el CCCB era visibilizar a los pacientes que utilizan cannabis con fines medicinales y a los cuidadores de estos pacientes. Sacarlos de las consultas, de la estigmatización en la viven en sus casas.
A los que estamos sanos siempre nos produce cierta vergüenza el exhibicionismo de una enfermedad pero no pretendíamos exhibir nada. Queríamos que los pacientes contaran sus historias de superación y que explicaran cómo con el cannabis están mejorando en su calidad de vida, en su autocuidado. Aprovechamos también este espacio para que médicos y pacientes se encontraran fuera del entorno donde se encuentran siempre, las consultas. El objetivo final es seguir empujando para que llegue una regulación y se instauren programas de cannabis medicinal.
¿Cómo está ahora la regulación? Habláis de copiar ideas del modelo de Holanda.
No es copiar a Holanda… Naciones Unidas en los convenios de 1961 fiscalizan el cannabis. Cuando crean la primera prohibición de estupefacientes incluyen tres plantas: la hoja de coca y la amapola del opio son las otras dos. Algo insólito porque lo da la naturaleza… las engloban dentro del término estupefacientes aunque farmacológicamente no tengan nada que ver nada una con otra.
Determinan entonces que el único uso lícito de los estupefacientes es el uso con fines médicos y científicos. Espanya en el 1967 ratifica mediante la ley de estupefacientes este convenio y mantiene los únicos usos lícitos. Para gestionar el comercio y la venta controlada se crea la Oficina de Control de Estupefacientes: controla la venta y la prescripción de estupefacientes como por ejemplo de la morfina o otros fármacos de uso cotidiano en medicina.
Tanto la legislación española como la internacional reconocen que el cannabis tiene usos médicos y científicos. De hecho en 2014, Naciones Unidas publica cuales son los requisitos que han de cumplir los países para aplicar programas de cannabis medicinal y no van más allá de lo dicho en 1961. Holanda fue el primer país en implantarlo. Ya hay decenas de países en el mundo de muy diferente condición económica y social que lo hacen. Desde Estados Unidos, donde hay más de 30 estados con leyes específicas para el uso medicinal y otras para uso recreativo, a Italia, Finlandia, Alemania, Canadá, Zimbabue…
Son programas en los que se dan licencias de cultivo de diferentes variedades de cannabis que tienen calidad farmacéutica que luego se venden en las farmacias para aquellas enfermedades donde los médicos crean que puedan ser beneficiosas.
No existe una cifra oficial sobre el uso del cannabis para usos terapéuticos, pero el Observatorio Español de Cannabis Medicinal habla de entre 50.000 y 100.000 personas que lo estarían utilizando. ¿Cómo se sabe si no está regulado?
No se puede saber. Los datos que nosotros manejamos los tenemos por trabajar con pacientes en asociaciones. Dependiendo del tipo de asociación, si es más comercial o menos, pero en las que tienen un fin más social hay entre un 6 y un 10% de socios que son terapéuticos. Pero de ahí a extrapolar una cifra…
Claro. Como no está regulado tampoco puede contabilizarse. ¿Se podría decir que las personas que hacen un uso medicinal se lo autorecetan?
Si. Quien lo toma se está automedicando. Hay médicos vinculados a asociaciones que lo aconsejan pero no prescriben ni recetan porque no pueden. Los pacientes que utilizan cannabis o bien lo cultivan o bien lo consiguen del mercado negro o bien lo obtienen de clubs cannábicos.
En España lo único que hay es el Sativex, que es un spray sublingual de uso hospitalario como segunda línea de tratamiento para la espasticidad asociada a la esclerosis múltiple y algunos usos compasivos como dolor neuropático o dolor oncológico. Esto depende del comité de ética de referencia. Hay personas que viviendo en la mismo ciudad tienen acceso y otras que no dependiendo de si el comité de ética lo ha autorizado o no. La realidad es esta: que la legislación reconoce el uso medicinal pero no se ha implementado.
La paradoja es que la Agencia Española del Medicamento ha otorgado licencias de cultivo de cannabis a algunas empresas con el fin que estas lo exporten a aquellos países con programas medicinales.
¿En qué consistiría el programa piloto que queréis empezar a implementar?
Primero pedir a la administración que de alguien con una licencia de cultivo y con conocimientos suficientes se extraiga una cantidad de cannabis y que se provea un tratamiento de cannabis a un número reducido de personas y se haga un seguimiento para ir poco a poco desarroyando un programa más general.
Tu has trabajo con personas que consumen cannabis con fines medicinales. ¿Cuáles son sus miedos, cómo dan el paso o cómo se deciden viendo que está fiscalizado?
Si autocultivas, si la policia te detecta las plantas te expones a unas multas que pueden ir de 6.000 a 8.000 euros. Si vas a la asociación y la policia te para por la calle y vas con una bolsita de marihuana pues también te expones a una multa de 600 a 6.000 euros. Hay una inseguridad jurídica bastante manifiesta. Y una indefensión. Además encontrándonos con la paradoja que las personas que utilizan cannabis, aunque no siempre el cannabis funcione para las enfermedades que tienen, mejoran su calidad de vida y también reducen la prescripción farmacéutica. Esto quiere decir que hacen un menor gasto sanitario tanto farmacéutico como en visitas al médico.
Se reduce la toma de ansiolíticos, de fármacos para dormir y de analgésicos. Incluso también de antidepresivos. Una enfermedad crónica se acompaña con un grado de ansiedad importante. Toda enfermedad crónica tiene unas repercusiones psicológicas para las que parece que el cannabis es beneficioso. No funciona para todas las personas pero a quien le funciona, le funciona muy bien.
El miedo que tendría el gobierno para no desarrollar esta regulación, ¿cuál sería?
Hay una guerra entre los diferentes partidos para ver quien se hace la foto. Ciudadanos apuesta por el cannabis medicinal exclusivamente. Podemos quiere una regulación integral que incluye también el cannabis medicinal. El PSOE vive en su esquizofrenia donde unos quieren y otros no y no se ponen de acuerdo y el PP tiene muy claro que no quiere una cosa ni otra. Aunque por otra parte el PP esta dando licencias a sus amigos para que cultiven marihuana para exportar fuera.
Luego hay unos problemas políticos que van a estar siempre por encima de la regulación del cannabis medicinal. Mientras no haya incidencia política suficiente para que los políticos consideren que es un problema que deben atajar no van a hacer nada. Vivimos en una situación de una insolidaridad extrema. Los partidos han perdido toda la responsabilidad que tienen con la ciudadanía.
A día de hoy, por lo menos en esta legislatura, no veo solución aunque las leyes ya están hechas, sólo se tienen que implantar.
¿Qué se tiene que hacer a nivel social para romper el estigma asociado al cannabis?
Que los pacientes sean visibles, que los médicos vayan tomando conciencia que hay mucha gente que consume cannabis con fines medicinales y que esto no se va a frenar. No se va a frenar porque son experimentos que si no hubieran funcionado no se habrían seguido expandiendo. El ejemplo es la proliferación en los diferentes estados de Estados Unidos. Debe estar funcionando.
A nivel social, desde ICEER intentamos visibilizar a los pacientes y tratar que se reconozca lo que estan haciendo y también dar información objetiva y científica sobre el uso del cannabis medicinal.
También debe haber una guerra dialéctica con médicos que no creen en el uso por no ser un fármaco propiamente dicho.
Claro. El problema del cannabis es que rompe con una regla básica de la medicina: tener un fármaco aislado que actúe de forma específica sobre los órganos diana y que el efecto terapéutico sea exclusivamente esto. Sabemos que es una fantasia, que los fármacos tienen efectos secundarios, que cuando se toca una cosa se alteran otras… Este es el juego de la farmacologia: tratar de ser cada vez más específicos y con menos efectos secundarios.
El cannabis rompe esto porque tiene más de cinco cannabinoides, más de 400 compuestos diferentes y rompe con esta especificidad y con esta pureza. El sistema cannabinoide es redundante, esto quiere decir que por diferentes vías se activan diferentes procesos. Para ellos es aprender un lenguaje nuevo.
¿En qué consiste este sistema?
El sistema endocannabinoide (SEC) es un sistema fisiológico básico que está en todo nuestro organismo que se encontró en los años 90 y que está en todos los mamíferos. Es un sistema encargado de mantener el equilibrio fisiológico. Muchas enfermedades tienen una alteración patológica del sistema endocannabinoide y utilizando cannabinoides exógeneres, bien por medio de la planta o mediante sintéticos, se puede ayudar a modular el sistema endocannabinoide en los procesos donde está involucrado.
Hay mucha adversión a las pseudoterápias o pseudociencias. ¿Se encaja el cannabis dentro de estos ámbitos?
El cannabis tiene esta peculariedad. Es un producto herbal con una evidencia científica clara pero no cumple con los criterios específicos de los fármacos aislados. Entonces está en una tierra incómoda para la ortodoxia. Creo que el momento que se empezó a conocer como funcionaba el cannabis… hablar de que el sistema endocannabinoide obedece a una pseudoterapia… pues la verdad, sería de una gran ignorancia.
A día de hoy, la literatura sobre el SEC, sobre propiedades terapéuticas del cannabis, sobre efectos de los programas de cannabis medicinal sobre la salud públcia es inabarcable.
Es una cuestión de haber relacionado siempre el cannabis al uso recreativo y por razones sociopolíticas haber ido acorralando la parte biomédica. Que en los 90 se descubriera el SEC es lo que hace que cada vez esté más aceptado dentro de la medicina. Sigue habiendo reticencias; probablemente los que más son los psiquiatras por el hecho que el cannabis produce psicoactividad. La verdad, los programas que hay y los estudios que se han hecho con cannabinoides, cuando hay un control médico, dan buenos resultados.
Y además después se dan fármacos psicoactivos como el metilfenidato a niños que se diagnostican de hiperactividad y estos son fármacos mucho más peligrosos que el cannabis. El miedo o respeto supongo que tiene que ver con el hecho de estar prohibido y de tener este estigma. Cada vez se va rompiedo más. Ya no es una cuestión de si ocurrirá o no ocurrirá, la cuestión es cuando.
